Sample collection and submission forms
All samples must be accompanied by the appropriate sample submission form. Samples accompanied by incomplete forms will not be accepted in accordance with NSW DPI Laboratory Services Terms and Conditions. For assistance completing these forms contact Customer Service.

Submission form
- Soil sampling kits are available from the NSW DPI Environmental Laboratory by contacting Customer Service on 02 66261103. You can also pick up kits from DPI and Local Land Service Offices or order online. Soil Sampling kits include a sample bag, sample submission form, information sheet and a pre-paid postage mail cylinder
- Soil sampling kits can be used for submission of one sample per kit. If you have multiple samples, please contact Customer Service or call the laboratory direct on 02 66261103 for a supply of bags and paperwork.
- Soil sampling kits are free to receive however there is a charge associated with the testing after the sample is sent to the laboratory and results are reported.

- Water sampling kit are available from the NSW DPI Environmental Laboratory by contacting Customer Service on 02 66261103. You can also pick up kits from DPI and Local Land Service Offices or order online. Water Sampling kits include a sample bottle, sample submission form, information sheet and a pre-paid postage mail cylinder
- Water sampling kits can be used for testing many water sources including: dam, creek, river, bore, well and tank water.
- Drinking water for Human consumption requires an additional analysis (E Coli and Total Coliforms). Special kits are required for these samples and can be obtained from NSW DPI Environmental Laboratory by contacting Customer Service on 02 66261103
- Water sampling kits are free to receive however there is a charge associated with the testing after the sample is sent to the laboratory and results are reported.
