Photograph release agreement

(If the applicant is under 18 years of age the photo must be registered under a parent or guardians name)

I (the entrant) consent to the Department of Industry and Investment for and on behalf of the State of New South Wales of 161 Kite Street Orange NSW 2800 ABN 51 734 124 190 (I&I NSW) using my photographs in relation to the Recreational Fishing Photo Competition and:-

agree to grant I&I NSW a Licence to use any copyright pertaining to photograph(s) supplied by me for the purposes of promoting the Recreational Fishing Photo Competition and for publishment in the Recreational Fishing Calender or any other Recreational Fishing promotional material and;
warrant that the copyright pertaining to photographs supplied by me is solely mine to licence and I indemnify I&I NSW in respect of any breach of this warranty;
warrant that I have obtained the consent of all persons depicted in the photograph(s) to use the photographs for the purposes of promoting the Recreational Fishing Photo Competition and for publication in the Recreational Fishing Calender or any other Recreational Fishing promotional material and I indemnify I&I NSW in respect of any breach of this warranty;
authorise I&I NSWto use and publish the photographs supplied by me, in any medium and in all forms, including in composite or altered form, alone or in conjunction with any wording or other photographs for any lawful purpose whatsoever;
authorise I&I NSW to use my name in connection with the photographs supplied by me, at its discretion;
authorise I&I NSW to supply the photographs to another government agency of the State of New South Wales for that agencys use without further notice or consent, provided that the photographs may only be used in accordance with this release; &
waive any right to inspect or approve the finished use of the photographs.
I have read this release and am fully familiar with and understand its contents. My details (name and address) and this Release are being provided by me voluntarily. I understand that:
- I&I NSW is collecting the photographs as part of a Competition where twelve successful applicants will have their photos published in the Recreational Fishing Calender.
- I&I NSW will use the photographs for the purposes of promoting the Recreational Fishing Photo Competition and/or Recreational Fishing.
- The photographs will be given to any party that obtains a Recreational Fishing Calender or accesses Recreational Fishing Photo Competition promotional material or any other Recreational Fishing promotional material.
- I do not have to provide my details but failure to do so may mean that I am not identified in material relating to the Recreational Fishing Photo Competition.
- The details and photographs will be held at I&I NSWs offices at 4 Marsden Park Road, Calala NSW 2340; and
- I can contact I&I NSW to access or correct the details at any time.

I&I NSW undertakes not to use the Material in a derogatory or otherwise inappropriate manner.
