Primary Schools - Get Hooked, It's Fun To Fish registration form

This agreement outlines the responsibilities of each party [NSW Department of Primary Industries (DPI)], and your school as a representative of NSW DEC in the delivery of the ‘Get Hooked… It’s Fun to Fish’ program.

As the registering school’s contact person, you agree to the following:

• Ensure that the contact person(s) and school details are correct and current on the registration form.
• Where possible, agree to take photographs of students participating in activities and provide a copy to the Get Hooked Staff (provided students have photo release consent)
• If requested, provide written, visual or verbal feedback to the Schools Manager, Get Hooked about.
  • The Get Hooked Teachers’ Manual (USB) and Resource kit contents used to deliver the codes/program in class.
  • Any issues of concern relating to volunteer assistance/timetabling.
  • Any media interest, and or parental/community comment in relation to the program.
  • Advise NSW DPI of media interest prior to any events associated with the Get Hooked program, including practical fishing sessions etc.

I have read the above conditions and am fully aware of my responsibilities as the schools contact and agree to abide by these guidelines.

NSW DPI Staff agree to the following in delivery of the ‘Get Hooked… It’s Fun to Fish’ program:

• Ensure that the registered school has the Get Hooked resources as per GH registration policy.
• Ensure that all NSW DPI staff and volunteers have undertaken the appropriate security checks (i.e. National Criminal History check and Working with Children Check) and have undertaken NSW DPI approved training prior to attending the school.
• Ensure regular contact with the registered school.
• Ensure all issues of concern relating to GH and volunteers are recorded and addressed.
• Endeavour to promote the program through local media and where possible assist with events associated with student’s involvement in the Get Hooked program.
• Ensure NSW DPI and NSW DEC policies are adhered to in relation to the program (i.e.media)
