Sample collection and submission forms

All samples must be accompanied by the appropriate sample submission form. Samples accompanied by incomplete forms will not be accepted in accordance with NSW DPI Laboratory Services Terms and Conditions. For assistance completing these forms contact Customer Service.

  • Olive oil sample collection kits  are available from NSW DPI Laboratory Services by contacting Customer Service. Olive oil sampling kits include a sample submission form, sample collection guide and pre-paid postage to the laboratory.

Water testing

Submission form

  • Water sampling kits are available from NSW DPI Laboratories by contacting Customer Service. Water sampling kits include a sample submission form, sample collection guide and pre-paid postage to the laboratory.
  • Note: Water sampling kits are for the testing of dam, creek, river, bore of irrigation water only. Water testing kits do not assess the suitability of water for human consumption.

Courier services and specimen delivery

NSW DPI utilises the services of the following nominated couriers:

  • Metrostate Couriers
  • StarTrack

Please contact the Customer Service for courier account details and to arrange pre-printed, pre-paid consignment notes. Couriers can then be contacted directly to make bookings and arrange collection of packages. Please contact Customer Service to arrange after-hours or same day deliveries

Commercial couriers may use either road or air transport and specimens should therefore be packed in accordance with International Air Transport Association (IATA) requirements. Most submission sent by veterinarians to the SVDL for testing are defined as “Biological Substance Category B” and must be packed according to the IATA packing instructions 650 (Biological Substance Category B). As requirements for transport can change, customers should consult with their transport agent to obtain current requirements.

Samples may also be hand delivered to the site during normal business hours.
